Effective safety management depends not only on identifying that something went wrong or didn’t produce the expected result but on truly understanding why and then being able to take effective and sustainable steps to prevent it from happening again. This course takes participants through the complete cycle of investigation, root cause analysis and corrective action planning, with an emphasis on practical application. Participants can expect to gain the skills and confidence to conduct investigations, identify root and contributing causes using proven tools, and develop corrective actions which lead to tangible results.

This course takes participants through the complete cycle of investigation, root cause analysis and corrective action planning, with an emphasis on practical application. Participants can expect to gain the skills and confidence to conduct investigations, identify root and contributing causes using proven tools, and develop corrective actions which lead to tangible results.

This course takes place over two half-days, in-person, at the Region of Waterloo International Airport’s Operations Centre located at 75 New Germany Lane in Breslau. The course will be in workshop format, with some modules of theory and others where you will put into practice what you have learned.

Learning Objectives

By the end of this course, participants will be able to:

  1. Describe the relationship between investigations, root cause analysis and effective corrective actions

  2. Identify common investigation pitfalls that can lead to incomplete or misleading findings

  3. Apply at least two RCA tools (including HFACS and Ishikawa) to realistic scenarios

  4. Distinguish between root causes, contributing factors and proximate causes

  5. Develop corrective actions that are specific, assigned, measurable and time-bound

  6. Describe methods for validating the effectiveness of corrective actions

  7. Explain how investigation findings feed back into an organization’s Safety Management System (SMS)
